Qualifications

• ACLS certification preferred.

• Knowledge of discharge planning processes required.
• Demonstrated ability to lead clinically and work in a multidisciplinary team.

Responsibilities

• Lead cooperative processes and organizational initiatives regarding patient throughput.
• Participate in multidisciplinary patient rounds to facilitate completion of the treatment/care plan.
• Prepare the patient for the next level of care.
• Demonstrate effective interpersonal and written/verbal communication with the care team.
