Qualifications

• BLS Provider within 30 days of hire. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from Indiana Board of Nursing before hire date.

Responsibilities

• Coordinate all aspects of the discharge process to ensure timely discharge.
• Identify barriers to discharge to facilitate patient throughput.
• Optimize bed availability and enhance the patient experience.
• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition as appropriate.
• Serve as primary coordinator of all disciplines for well‑coordinated patient care.
• Note and carry out physician and nursing orders.
