• Experience with electronic medical records documentation.

• Conduct resident assessments and update care plans regularly.
• Develop, implement and evaluate person-centered care plans with families and team.
• Ensure regulatory compliance with state and federal standards.
• Oversee medication management and audits for safety and accuracy.
• Provide leadership, mentorship and staff education on nursing protocols.
• Foster collaboration with residents, families, staff and external providers.
• Coordinate with physicians, therapists and hospitals to ensure continuity of care.
• Apply critical thinking to emergencies and dynamic health situations.
• Support quality improvement initiatives and monitor KPIs.
• Participate in on-call rotations after hours and weekends.
