Job summary

Sovah Health - Danville is seeking an RN Director of Case Management to lead departmental strategy and operations in an in-person setting. The role focuses on discharge planning coordination among RN and social work teams to ensure high-quality patient care and regulatory compliance.

The ideal candidate will have an active Virginia RN license (or compact state), a Bachelor’s in Nursing, and prior case management leadership experience.

Qualifications

• RN license in VA or compact state required.
• Bachelor's degree in nursing required.
• Previous Case Management experience required.

Responsibilities

• Develops and implements departmental goals, plans, and standards aligned with organizational requirements.
• Directs and evaluates departmental operations, including patient care delivery and service levels to meet quality objectives.
• Plans staffing activities, including hiring, orientation, evaluation and continuing education initiatives.
• Prepares and monitors departmental budgets and coordinates audits.
• Creates an environment that encourages professional growth and development.
• Integrates evidence-based practices into operations and clinical protocols.

Our Case Management team focuses on serving the adult population in our community, maintaining a patient-to-staff ratio that is below the national average. Our collaborative approach to safe, effective discharge planning includes close coordination between a Utilization Review RN, Case Manager RN, and Social Worker-ensuring each patient receives comprehensive, personalized support throughout their care journey.

How you'll contribute

• Develops and implements departmental goals, plans, and standards consistent with the clinical,administrative, legal, and ethical requirements/objectives of the organization.
• Directs and evaluates departmental operations, including patient care delivery, information technologies, service level determination, and complaint management, to achieve performance and quality control objectives.
• Plans and monitors staffing activities, including hiring, orienting, evaluating, disciplinary actions, and continuing education initiatives.
• Prepares, monitors, and evaluates departmental budgets, and ensures that the department operates in compliance with allocated funding. Coordinates and directs internal/external audits.
• Creates and fosters an environment that encourages professional growth.
• Integrates evidence-based practices into operations and clinical protocols.

Annual range - $83,616 to $112,881.60 annually
