Sovah Health - Danville is seeking a Director of Case Management to lead our team in delivering patient-centered care. You will direct departmental operations, manage staffing and budgets, and drive evidence-based practices across the unit in a collaborative setting.
The role requires an RN license (VA or compact), a bachelor’s degree in nursing, and prior case management or leadership experience. This is an in-person, day-shift opportunity at Sovah Danville.

Our Case Management team focuses on serving the adult population in our community, maintaining a patient-to-staff ratio that is below the national average. Our collaborative approach to safe, effective discharge planning includes close coordination between a Utilization Review RN, Case Manager RN, and Social Worker-ensuring each patient receives comprehensive, personalized support throughout their care journey.

Annual range - $83,616 to $112,881.60 annually
