Job summary
MCR Health, Inc is seeking an experienced Registered Nurse Diabetic Educator in Bradenton, FL to provide comprehensive diabetes self-management education and support to patients with diabetes and pre-diabetes. You will work with physicians, nurses, and community resources to improve outcomes and encourage healthy lifestyle changes.
The role requires an active RN license in Florida, BLS certification, and at least two years of clinical nursing experience.
Qualifications
- Minimum of two years of clinical nursing experience.
- Experience in diabetes management or related fields preferred.
- BSN preferred; ADN required.
- Current RN license in the applicable state.
- Current BLS certification required.
- CDCES preferred or required within a specified period of employment.
Responsibilities
- Provide comprehensive diabetes self-management education and support to patients.
- Collaborate with providers, care teams, and community resources.
- Promote healthy lifestyle changes and disease prevention.
- Ensure services meet professional standards of care.
- Support diabetes prevention and management programs.
