The RN Diabetic Educator will work with care teams, assess learning needs, develop education plans, and document care while upholding safety and professional standards in a patient-focused environment.

Qualifications

• ADN/AON required.
• Current RN license in the applicable state.
• Minimum 2 years clinical nursing experience.
• BLS certification required.
• Strong communication and patient education skills.

Responsibilities

Location: Bradenton, FL 34205 Pay Rate: $29.19-$36.49 per hour Employment Type: Full-Time, Hourly Industry: Healthcare / Non-Profit / Education

Position Overview

We are seeking a compassionate and experienced Registered Nurse (RN) Diabetic Educator to provide comprehensive diabetes self-management education and support to patients living with diabetes, pre-diabetes, and related metabolic conditions.

The RN Diabetic Educator will work closely with healthcare providers, care teams, patients, and community resources to promote healthy lifestyle changes, improve patient outcomes, and support effective disease prevention and management.

This position is ideal for an RN with strong clinical experience and a passion for patient education, chronic disease management, and improving quality of life.

Key Responsibilities

• Provide individualized diabetes education and self-management support to patients with diabetes and pre-diabetes.
• Educate patients on medication management, nutrition, exercise, glucose monitoring, and lifestyle modifications.
• Assess patients' learning needs, health goals, and barriers to effective diabetes management.
• Develop and deliver education plans based on individual patient needs.
• Reinforce provider treatment plans and help patients understand their role in managing their health.
• Monitor patient progress and communicate relevant findings and concerns to providers and care teams.
• Collaborate with healthcare providers, interdisciplinary teams, and community resources.
• Support disease prevention, chronic disease management, and improved health outcomes.
• Maintain accurate and timely patient documentation in accordance with professional and organizational standards.
• Provide high-quality, timely, and compassionate patient care.
• Promote patient engagement, self-management, and healthy lifestyle behaviors.
• Maintain compliance with applicable clinical, safety, and professional standards.
• Participate in quality improvement and other departmental initiatives as needed.

Required Qualifications

• Associate Degree in Nursing (ADN/AON) required.
• Current Registered Nurse (RN) license in the applicable state.
• Minimum of 2 years of clinical nursing experience.
• Current Basic Life Support (BLS) certification required.
• Strong communication, organization, and patient education skills.
• Ability to work effectively with patients, providers, and interdisciplinary teams.

Preferred Qualifications

• Experience in:
  • Diabetes management
  • Endocrinology
  • Primary care
  • Community health
  • Chronic disease management
• Certified Diabetes Care and Education Specialist (CDCES) certification preferred, or willingness to obtain certification within the required employment timeframe.
• Additional certifications related to chronic disease management are a plus.
• BSN preferred.
