Cleveland Clinic Weston Hospital in Weston, FL, is seeking an RN Diabetes Patient Educator to coordinate the diabetes education program, provide resources, and improve patient care through education and collaboration.

The role requires a nursing or related degree, Florida licensure, and CDCES within two years. You will team with Endocrinology to design care plans and deliver diabetes self-management education.

Qualifications

• Bachelor's degree or higher in Nursing, Dietetics, Pharmacy, or a closely related field.
• One year of experience as a Registered Dietitian, Registered Nurse, or Pharmacist.
• Current license to practice dietetics, nursing, or as a Pharmacist in the State of Florida.
• Active certification as a Registered Dietitian/Nutritionist (RD/RDN) for Dietitians.
• Certification as a Certified Diabetes Care and Education Specialist (CDCES) within two years of hire/assignment.

Responsabilités

• Provide direct patient care using medical nutrition therapy or diabetes self-management education.
• Collaborate with Endocrinology providers to design and administer nutrition and diabetes care.
• Complete comprehensive assessments for each patient, including emotional and behavioral health.
• Develop individualized care plans with safe and effective interventions.
• Utilize national standards and guidelines for diabetes and obesity care.
• Use diabetes and weight management technology to assess status and track outcomes.
• Deliver specialized interventions and comprehensive counseling services.
• Implement care plans, communicate plans, and maintain documentation.
• Provide timely documentation of each patient encounter in the electronic medical record.
• Develop and contribute to education programs for healthcare professionals and the public.
