Job summary

Cleveland Clinic Weston Hospital is seeking an RN Diabetes Patient Educator to coordinate diabetes education, provide resources to physicians and patients, and promote evidence-based care.

You will work days 8:00 a.m. - 5:00 p.m. in the Endocrinology-Weston Hospital department, collaborating with endocrinology providers to design nutrition and diabetes education plans.

Qualifications

• Bachelor's degree or higher in Nursing, Dietetics, Pharmacy or closely related field.
• Dietitians must have completed a dietetic internship through a credentialed program.
• Maintain current license to practice in Florida (Nursing, Dietetics or Pharmacy).
• RD/RDN certification active.
• CDCES certification within two years of hire.
• One year of experience as a Registered Dietitian, RN or Pharmacist.

Responsibilities

• Provide direct patient care using medical nutrition therapy or diabetes self-management education for assigned patient populations.
• Collaborate with Endocrinology providers to design and administer nutrition and diabetes care and education.
• Complete comprehensive assessments including emotional health, knowledge level, disease history, and EMR data.
• Develop individualized care plans with safe interventions and promote self-management.
• Utilize national standards/guidelines for diabetes care, including ADCES7 Self-Care Behaviors.
• Use diabetes and weight management technology to assess status, provide care, and track outcomes.
• Deliver counseling and education for patients with diabetes and obesity needing nutritional management.
• Implement care plans, communicate plans, establish outpatient follow-up, and maintain documentation.
• Provide complete, concise, timely documentation of each encounter in EMR.
• Develop education programs for physicians, nurses, allied health professionals, students, and the public.
• Create/revise education materials using evidence-based guidelines.
• Participate in program development to market services and drive revenue.
• Market services to referral sources.
• Develop and implement services in response to patient/provider needs.
• Complete productivity and outcomes reports.
• Adhere to policies, standards, safety, and regulatory requirements.
