About This Role

As an RN Care Coordinator in Gastroenterology, you will collaborate with multidisciplinary caregivers to provide longitudinal coordination of care and disease management for patients with chronic conditions. This role is ideal for those who excel in patient advocacy and care coordination.

What You’ll Do

• Work collaboratively with a multidisciplinary care team for high-risk patients to develop goals and plan interventions.
• Provide care and disease management coordination.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Outline the nature and duration of involvement needed by the specialty care team.
• Utilize assessment skills and risk assessment tools to identify patients with care needs.
• Conduct comprehensive clinical assessments including medical, behavioral, and social needs.
• Inform and work with patients and families regarding care coordination.
• Assist in managing transitions of care across settings.
• Identify barriers, facilitate solutions, and connect to community resources.
• Educate patients and families on managing specialty or surgical conditions.

Requirements

Education

• Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

Experience

• 3-5 years of nursing experience
• Prior case management experience preferred

Licenses & Certifications

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) certification required
