Avera Diabetes Services-Sioux Falls is seeking an RN to provide diabetes self-management education to patients and support the nursing staff with diabetes care. The role involves planning and evaluating education programs, and collaborating with system-wide diabetes resources.

The ideal candidate will have an active RN license and 4–6 years of disease management experience, with a passion for diabetes education and adapting to new technologies.

Qualifications

• Registered Nurse (RN) with active license in the state of practice.
• Minimum 4–6 years of experience in disease management or case management preferred.
• Experience in diabetes management is desirable.

Responsibilities

• Provide individual and group diabetes self-management education.
• Serve as a clinical resource for providers and nursing staff on diabetes care.
• Develop and evaluate training programs for clinic staff in diabetic education.

Work where you matter.A Brief OverviewProvides individual and group diabetes self-management education, conducts community and health professional education related to diabetes, and serves as a resource for the nursing staff relative to diabetes education and basic nursing care of the diabetes patient.What you will doPlans, conducts and evaluates system diabetes disease management processes and systems.Serves as a clinical resource person for providers and nursing staff related to the care and education of diabetes patients.Supports clinics in disease management and access to diabetes education.Identifies opportunities for program development and participates in special projects as assigned.Develops, conducts, and evaluates training program for clinic staff to provide diabetic education.Collaborates with diabetes management and education resources across the system.Maintains expert knowledge in diabetes education and case management tactics.Essential QualificationsThe individual must be able to work the hours specified.
