Job summary

The Health Coach at Mainline Health Systems, Inc. works with the Diabetes Self-Management team to integrate clinical and self-management elements of diabetes and cardiometabolic care, aiming to optimize quality care for patients in a Patient-Centered Medical Home model.

Under the supervision of the Director of Health Education, the role emphasizes education, care planning, and collaboration with the care team to support individualized behavioral goals and continuous quality improvement.

Qualifications

• RN with at least 1 year of related experience.
• Current RN license in Arkansas.
• BLS certification required.
• Willingness to pursue continuing education in health education.

Responsibilities

• Coordinate communication between patients, families, and caregivers with the multidisciplinary care team.
• Integrate knowledge of cardiometabolic care and diabetes self-management into clinical practice.
• Deliver diabetes self-management education to individuals and groups.
• Document all patient contacts in the EHR following guidelines and timelines.
• Collaborate with community partners to establish and maintain ongoing support options.
