• Experience: One year of experience as a Registered Dietitian or Registered Nurse.
• Licenses & Certifications: Current license to practice dietetics or nursing in the State of Ohio; CDCES upon hire or within two years; Dietitians must have completed a dietetic internship and have active RD (RN) or Nutritionist (RDN) certification.

• Assist with the overall coordination of the diabetes education program.
• Provide educational resources to physicians, caregivers, and patients.
• Facilitate the organizational change process to promote and implement services.
• Provide direct patient care using medical nutrition therapy or diabetes self-management education.
• Collaborate with Endocrinology providers to design and administer nutrition and diabetes care.
• Complete comprehensive assessments for each patient, including emotional and behavioral health.
• Develop individualized care plans with safe and effective interventions.
• Utilize national standards and guidelines for diabetes and obesity care in practice.
• Deliver specialized interventions and comprehensive counseling and education services.
• Implement care plans, communicate plans, and maintain documentation.
