Job summary

Bethesda Health Group is seeking a Registered Nurse for resident assessment and care planning in our facility in Belleville, IL. The role requires accurate documentation, collaborative care planning, and timely communication with physicians and families.

The RN will administer medications, treatments, and IV therapies, supervise interventions, and respond to changes in condition using SBARR, while maintaining high standards of patient safety and quality care.

Qualifications

• Education from an accredited school of nursing resulting in RN.
• Current RN license in the state you will be working.
• CPR/AED certification required or willing to obtain within six months.
• Long-term care experience preferred.
• Must possess critical thinking skills and strong communication abilities.

Responsibilities

• Accurately assesses residents' health status on admission and ongoing.
• Develops dynamic care plans with an interdisciplinary team.
• Documents the care plan and interventions accurately.
• Performs nursing interventions including medications, treatments, and IV therapies.
• Communicates the plan to IDT members, residents, and families.
• Uses SBARR for timely physician notification during changes in condition.
• Assists with ADLs and responding to call lights.
• Maintains safety and quality through QAPI process.
