Job summary

The Registered Nurse (RN) on the CTI and Transitional Residential Setting (TRS) Team supports adults with behavioral health needs as they transition from hospitals into the community. The RN provides CTI care management and health services within the nursing scope, coordinating with mental health professionals, care managers, hospitals, and community-based providers to ensure continuity of care.

This split position covers community and TRS housing; travel is required across Nassau and Suffolk

Qualifications

  • Education: Associate’s degree in nursing required; bachelor’s degree preferred.
  • Experience: Minimum of two (2) years RN experience; community mental health or related experience preferred.
  • Specialized Training/ Licenses: Valid NYS RN license; CTI certification within 12 months; 9.58 designation within 6 months.

Responsibilities

  • Provides CTI services across engagement, linkage, monitoring, and transfer of support to a caseload of 30 adults.
  • Conducts health assessments, monitors physical health needs, and addresses urgent medical issues.
  • Accompanies individuals to medical and psychiatric appointments and ensures follow-up.
