A leading health system in Detroit seeks a Nurse Coordinator responsible for developing and implementing patient care programs. This role requires collaboration with healthcare teams to ensure high-quality outcomes and patient education. Candidates should have a strong nursing background with preferred certifications and extensive clinical experience.
GENERAL SUMMARY:
*Reports to the Nursing Leader/Administration. The Nurse Coordinator is a Registered Nurse who under the direction of department leadership develops, implements, monitors and evaluates a program of care for patients.
*Coordinates care across the continuum to facilitate quality outcomes.
*Collaborates with other healthcare team members including physicians, nurses, extenders, ancillary and support staff to ensure optimum patient care and satisfaction.
*Provides education to patients and family regarding disease process. Trains and directs a nursing team in the optimal care of the patient.
EDUCATION/EXPERIENCE REQUIRED:
*Graduate from an accredited school of nursing.
*Successful completion of HFHS (Parent Operating Group) Pharmacology test.
*Maintains and demonstrates current clinical knowledge in area of specialty. Bachelor's Degree in Nursing preferred.
*A minimum of three to five years of progressively more responsible clinical nursing practice in specialty.
*Certification in specialty preferred.
*Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.
*Ability to perform a wide variety of professional nursing services.
*Ability to effectively communicate with patients, families, and medical group personnel.
*Preferred experience in the specialty.
*Basic computer knowledge and keyboarding skills. Knowledge of Word and Excel.
CERTIFICATIONS/LICENSURES REQUIRED:
*Current licensure by the Michigan State Board of Nursing as a Registered Nurse. Current BLS or ACLS per HFHS Policy.
