Qualifications

• RN Licensure in Michigan.
• Associate's Degree in Nursing required.
• BSN preferred but not required.
• Minimum 5 years of clinical nursing experience.
• Knowledge of provider community and community resources in White Cloud area.
• Comprehensive knowledge of insurance principles and outpatient delivery systems.

Responsibilities

• Coordinate care for a group of patients identified using MiPCT guidelines.
• Identify targeted high-risk populations per PCP referral and risk stratification.
• Manage a caseload of ~150 complex patients, with 30–50 actively engaged.
• Assess health care, education, and psychosocial needs using standardized tools.
• Collaborate with PCP to develop and update a comprehensive plan of care in NextGen EHR Care Management template.
• Provide targeted interventions to avoid hospitalizations and ER visits.
• Coordinate care across settings and help patients/families understand health care options.
• Provide self-management support to empower patients/families.
• Infection prevention: develop, implement, and revise policies and procedures.
• Oversee infection control measures and ensure regulatory compliance including CDC guidelines.
• Complete post-hospital follow-up with med reconciliation and PCP scheduling.
• APIC certification completed; serve as infection control resource.
