Job summary

Mercy Home Health is seeking a registered nurse to deliver care through the nursing process, collect data, assess needs, plan and evaluate outcomes. You will coordinate patient teaching and collaborate with a health team to ensure safe, effective care across departments.

You will administer medications safely, assess progress, and contribute to quality improvement efforts while serving as a patient advocate and maintaining professional standards.

Qualifications

• Active nursing license in state of practice.
• Associate degree in nursing or diploma in nursing; BSN preferred.

Responsibilities

• Perform initial and ongoing patient assessment with the care team.
• Develop and implement care plans; administer medications safely.
• Coordinate patient teaching and ensure care continuity during transitions.
• Act as patient safety advocate and participate in quality improvement.
