Job summary
Thresholds is seeking a Registered Nurse to provide medical and psychiatric support in the community setting, based in Chicago, IL. You will collaborate with clinical teams to implement care plans, conduct assessments, and support clients on their recovery journey.
You will administer medications, schedule appointments, coordinate care, and offer health education to clients. This role involves home visits to remove barriers to healthcare and occasional emergencies, with a collaborative culture
Qualifications
- Registered Nurse license required.
- Associate's/Bachelor's/Master's in Nursing from an accredited program.
- Valid driver's license and daily access to own car.
Responsibilities
- Develop and implement care plans with clinical teams.
- Assess and monitor clients' health status and recovery.
- Administer medications and coordinate appointments.
- Provide health education and transportation to clients as needed.
- Document medical care and respond to emergencies or crises.
