Community Options, Inc. in Santa Fe, NM is seeking a full-time Registered Nurse to provide nursing care to individuals with disabilities in a community setting, following the nursing process and collaborating with health team members.

Ideal candidates hold a Bachelor’s degree or equivalent nursing experience, a valid RN license, and at least one year of direct nursing experience with persons with disabilities; a valid driver’s license is required and background checks will be conducted.

Qualifications

• Bachelor’s degree or equivalent experience in nursing.
• Minimum of one year of nursing experience with individuals with disabilities.
• Valid RN license.
• Valid driver’s license with satisfactory driving record.
• Background checks required.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by practice standards.
• Ensure individuals attend medical appointments per their ISP.
• Create and distribute medical/health reports detailing health goals and physician recommendations.
• Conduct trainings on medications administration and delegation.
• Develop and update ISP/PCP with interdisciplinary team including concerns and observations.
• Train employees on the medical needs of assigned individuals.
• Monitor health conditions and refer for medical services as needed.
• Maintain medication administration logs and monitor usage.
