Job summary

Apex Home Health, Hospice & Home Care is seeking a licensed RN to provide professional nursing care within an interdisciplinary team, applying the nursing process to assess, plan, implement, and evaluate patient care to achieve optimal outcomes.

You will assess patients, develop care plans, document accurately, educate patients and families, operate equipment, and ensure safety and confidentiality while collaborating to deliver high‑quality care.

Qualifications

• Licensed RN with current Illinois registration.
• Certifications in CPI and BLS (within specified timeframes)
• Diploma in Nursing
• Strong reading, writing, and oral communication skills
• Basic computer proficiency, comfortable with Windows OS
• Ability to maintain confidentiality and demonstrate professionalism

Responsibilities

• Assess patient status through interviews, observations, and physical exams
• Develop and modify individualized care plans collaboratively with patients and the healthcare team
• Implement interventions to meet patient goals and evaluate progress continually
• Ensure accurate documentation and maintain current knowledge of evidence-based practices
• Operate and troubleshoot medical equipment, perform bedside lab testing, and ensure quality control
• Coordinate interdisciplinary care, prioritize tasks, and communicate effectively within the team
• Provide patient education tailored to age, cultural, and developmental needs, promoting safety and understanding
• Advocate for patients and utilize resources to resolve clinical issues efficiently
