Job summary

Apex Home Health, Hospice & Home Care is seeking a compassionate Registered Nurse to join our multidisciplinary team in providing high-quality care to patients in familiar settings. You will assess patients, update care plans, supervise CNAs and LPNs, and collaborate with an interdisciplinary team to deliver evidence-based nursing services with empathy and professionalism.

Qualifications

• Graduation from an accredited nursing program (associate’s or bachelor’s degree).
• Current active RN license in the applicable state.
• One year of skilled nursing experience (preferred).
• CPR certification, either obtained at hire or during orientation, with ongoing current certification.

Responsibilities

• Providing quality nursing care to patients to promote their physical, mental, and psychosocial well-being.
• Conducting ongoing patient assessments and updating care plans based on condition changes.
• Offering clinical supervision to CNAs and LPNs.
• Implementing department policies, goals, and procedures.
• Ensuring excellent patient service and maintaining a positive attitude.
• Collaborating effectively within an interdisciplinary team.
