Job summary

Allegheny Health Network is seeking Registered Nurses to work in a high-volume call center, serving as the primary contact for physicians and referring providers seeking patient transfers within AHN. You will triage complex requests (telemedicine, consults, transfers, emergency alerts) and coordinate timely patient care transitions with diverse clinical teams.

Responsibilities include documenting verbal reports, reviewing EHR and labs, prioritizing bed allocation, and guiding non-clinical staff

Qualifications

• Required Associates or Diploma in Nursing or relevant experience and/or education as determined by the company in lieu of bachelor's degree.
• 5 years of experience in Clinical Nursing
• 3 years of experience in stepdown/critical care/Emergency Room
• Current State of PA RN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
• Preferred Bachelor's degree in Nursing, strongly preferred

Responsibilities

• Coordinate all aspects of patient care for adult and pediatric transfers, working collaboratively with the interdisciplinary Transfer Center team.
• 25% Facilitate patient throughput within and into the AHN network by, coordinating communication between referring facility providers and the accepting AHN providers and clinical management team.
• 20% Identify and prioritize the needs of incoming callers to provide patient-centered support, guiding them through the transfer process with education, screening, referrals, care coordination, and any additional assistance required
• 10% Simultaneously utilize several software platforms and a multi-channel phone system to maintain accurate patient records, coordinate timely access to care, and manage seamless communication between various clinical departments and external facilities.
• 15% Clinically prioritize and coordinate patient placement within the AHN network, ensuring optimal allocation of resources based on patient acuity, bed capacity, and staff availability
• 10% Provide clinical direction and support to non-clinical staff to enhance hospital efficiency, including but not limited to: prioritizing inbound patients, coordinating bed allocation, and facilitating timely patient placement.
• 10% Monitor department patient boards, prioritizing bed placement for critical or high-priority transfer patients. Receive and document updates on patients' clinical conditions within the electronic health record (EHR), maintaining continuous monitoring until the transfer request is finalized.
• 10% Other duties as assigned or requested.
