Job summary

Qualifications

• Associates or Diploma in Nursing required or equivalent experience.
• 5 years of experience in clinical nursing.
• 3 years in stepdown/critical care/emergency room.

Responsibilities

• Coordinate all aspects of patient care for adult and pediatric transfers with the Transfer Center team.
• Facilitate patient throughput by coordinating with referring facilities and AHN providers.
• Identify and prioritize callers' needs, guiding them through the transfer process.
• Utilize multiple software platforms and a multi-channel phone system to maintain patient records.
