Elara Caring is seeking a Clinical Team Specialist to oversee patient care across home health settings. You will coordinate patient care, referrals, and ensure ongoing assessment and updates to the plan of care while maintaining regulatory compliance.

The role requires an RN license, 2 years of home care experience in Home Health or Hospice, and the ability to travel between patient homes as needed. Expect a comprehensive benefits package and opportunities for career growth.

Qualifications

• Associate degree in Nursing required.
• 2 years home care experience as a registered nurse in a Home Health or Hospice environment.
• Current, unrestricted RN license valid for the state of work.
• Proficiency in Homecare Homebase, Workday, and Microsoft Office.

Responsabilités

• Oversee patient care services, coordinate patient care and referrals, and ensure ongoing assessment and updates to the plan of care.
• Review and approve plans of care and evaluate changes for clinical appropriateness.
• Perform regular OASIS reviews for all patients.
• Ensure required visits are completed timely.
• Review plans of care and paperwork for accuracy and regulatory compliance.
• Monitor documentation timeliness and maintain records in compliance with licensing and regulations.
• Participate in strategic planning and QAPI activities.
• Perform other duties/projects as assigned.
