Job summary

Apreva Hospice in San Diego, CA is seeking a skilled RN Clinical Supervisor to oversee the hospice team, coordinate services, and guide clinical decisions to ensure high-quality patient care.

The ideal candidate will bring RN licensure, preferred BSN, and at least two years of supervisory experience in hospice or home care, with strong leadership, communication, and documentation skills.

Qualifications

• Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation.
• BSN preferred.
• Minimum of two years of management or supervisory experience.
• Hospice or home care experience preferred.
• CPR certification.
• Strong communication and nursing judgment in an interdisciplinary team setting.

Responsibilities

• Review new patient referrals and assign appropriate hospice staff.
• Guide clinical staff through home visits, case reviews, and real-time consultations.
• Develop patient care plans and set treatment goals.
• Lead team conferences for coordinated, holistic care.
• Perform regular chart audits and provide quality feedback.
• Assist in hiring, onboarding, and training of clinical staff.
• Support staff development and continuing education programs.
• Contribute to policy development and uphold standards.
• Engage in community outreach to promote hospice services.
• Support quality improvement initiatives and organizational goals.

Qualifications

• Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation.
• Graduate of an accredited school of nursing; BSN preferred.
• Previous experience in hospice or home care setting required.
• Minimum of two (2) years of management or supervisory experience preferred.
• Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation.
• Must be a licensed driver with an automobile that is insured per state and/or organizational requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.
• Proven ability to work effectively within an interdisciplinary team setting.
• Demonstrates excellent observation, nursing judgment, and communication skills—both verbal and written.
• Understands the hospice philosophy and the emotional, clinical, and ethical considerations surrounding end-of-life care.
• Knowledge of pain and symptom management principles.
• Adheres to accepted professional standards and practices.
• Commitment to clinical excellence and thorough documentation.
