Assisted Home Health & Hospice in Culver City is seeking an RN Clinical Supervisor to lead our home health team, balancing clinical oversight with hands-on support across the agency.

You will supervise staff, coordinate care, and drive quality outcomes while ensuring patient-centered care and compliance with Title 22 and CMS requirements. A California RN license and leadership experience are essential.

Qualifications

• Active California RN license.
• 2+ years of recent home health or related clinical experience.
• Proven leadership or supervisory experience preferred.
• Strong clinical judgment and knowledge of home health regulations (Title 22, CMS).
• Excellent communication, organization, and team leadership skills.

Responsibilities

• Lead daily clinical operations, including staffing, scheduling, and care coordination.
• Supervise and mentor nurses, home health aides, and interdisciplinary team members.
• Ensure all patient care aligns with physician orders, care plans, and regulatory standards.
• Review and audit clinical documentation for accuracy, compliance, and quality.
• Partner with physicians and care teams to deliver seamless, coordinated care.
• Support onboarding, training, and ongoing staff development.
• Drive performance improvement initiatives and participate in QAPI programs.
• Provide clinical support in the field as needed, including complex patient cases.
• Step in as a leadership resource in the absence of the Director of Patient Care Services.

We’re seeking a skilled and motivated RN Clinical Supervisor to help lead our home health team in Culver City and ensure exceptional, patient-centered care. In this role, you’ll balance clinical oversight with hands‑on support, guiding staff, coordinating care, and driving quality outcomes across the agency.

The annual base salary range for this position at commencement of employment is expected to be between $110,000 – 125,000; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.
