Job summary

VNA & Hospice of the Southwest Region in Vermont seeks an experienced Registered Nurse Clinical Supervisor to oversee home health and hospice staff in Franklin County. You will coordinate referrals, orient new RNs, review plans of care, and ensure CMS compliance while fostering open communication and staff development.

Qualifications

• Graduation from an accredited school of nursing is required.
• Vermont RN license required; driver's license; background checks.
• Five years in home health clinical care; at least 2 years in a supervisor role.

Responsibilities

• Provides day-to-day oversight of home care and hospice staff.
• Oversees orientations and develops staff through coaching and performance plans.
• Ensures plans of care review quality and CMS compliance.
• Schedules and leads monthly team meetings and debriefings.
• Serves as a field RN as needed to meet operations.
• Maintains HR documentation and onboarding with preceptors.
