Job summary

VNA & Hospice of the Southwest Region in Vermont seeks a collaborative, innovative leader to promote and coordinate Vermont's Home Health & Hospice Nurse and Clinical Residency Program. This statewide role supports recruitment, development, and retention of new home health and hospice nurses across VT.

The position is grant-funded through the Rural Health Transformation Fund for up to five years, with possible extension, and starts around July 1, 2026.

Qualifications

• Bachelor's Degree required.
• Three years' experience in Medicare certified home health care required.
• Current Vermont State Registered professional disciplinary license preferred.
• Excellent interpersonal, communication and analytical skills.
• Demonstrated ability to work collaboratively with interdisciplinary teams toward a common purpose, balancing needs of individual participants.
• Has command of the principles of adult learning and demonstrates ability to use these in developing education plans.
• Software skills: Electronic medical record, Microsoft Excel, Word, Outlook, PowerPoint, etc.
• Valid State Driver's license required. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.

Responsibilities

• Develops, implements and evaluates a clinical education program for new nurses or nurses transitioning to professional practice in home-based care.
• Develops welcome packets and educational materials for residents and onboarding/ residency program packet for agencies.
• Identifies education trends and applicable adult learning strategies for program design and evaluation.
• Coordinates program needs assessment and oversees development of educational materials using instructional design for adult learners.
• Reviews resources and develops a Residency program guide with onboarding kit for agencies to adapt.
• Develops marketing materials that can be rebranded by VNAs of Vermont member agencies.
• Builds relationships with nurse education institutions and professional organizations to promote the residency program.
• Coordinates peer support groups and develops feedback for participants (mid-program and exit surveys).
• Provides programmatic updates to VNAs of Vermont Board in coordination with association staff.
• Collects workforce data for VNAs of Vermont Board and staff.
• Assists with grant tracking and reporting to State of Vermont.
• Performs other duties as required.
