Job summary

VNA & Hospice of the Southwest Region in Vermont is seeking an Intake Registered Nurse (RN) to manage timely assessment and processing of referred clients for home health and hospice services. The role follows a Sunday–Thursday schedule, 8am–4pm, with a focus on timely response, documentation, and coordination with physicians and referral sources.

The ideal candidate will have BSN preferred, relevant home health/hospice experience, and strong communication and team skills.

• Must be a team player with strong collaboration abilities.
• Ability to coordinate intake processes and communicate with providers and families.

Responsibilities

• Assess referred clients and initiate home health and hospice intake processes.
• Coordinate with scheduling, billing, and quality teams to ensure compliant referrals.
• Ensure all required documentation is completed in a timely manner according to CMS and agency guidelines.
• Support and facilitate patient care coordination across the continuum of services.
