Job summary

KeystoneCare is seeking an experienced Clinical Supervisor to oversee our clinical team across home health, hospice, and palliative services. The role focuses on leadership, mentorship, and operational oversight to ensure high-quality patient care and regulatory compliance.

You will support onboarding, coordinate care with interdisciplinary teams, review plans and records for accuracy, and drive quality improvement initiatives in a fast-paced healthcare environment.

Qualifications

• RN license in PA is required and must be active.
• 2–5 years of clinical experience in home health, hospice, or palliative care preferred.
• Supervisory or leadership experience preferred.

Responsibilities

• Supervise and support nursing and clinical staff, ensuring quality and consistency of patient care.
• Review and approve patient care plans, documentation, and clinical records for accuracy and compliance.
• Provide coaching, training, and performance feedback to clinical team members.
• Ensure compliance with state, federal, and accreditation standards (e.g., Medicare, CHAP).
• Collaborate with interdisciplinary teams to ensure coordinated and effective patient care.
• Participate in quality improvement initiatives and clinical audits.
• Serve as a clinical resource for staff, patients, and families.
• Support onboarding and orientation of new clinical staff.
