You'll help shape decisions that elevate both patient outcomes and the future of nursing.

Job Summary And Qualifications

The Clinical Support Resource Nurse demonstrates advanced knowledge, teaching skills and advanced nursing practice. To work in collaboration with the Executive Medical Director of Population Health, Administrative Director of Population Health as well as the Senior Analyst in Population Health to achieve Population Health goals and objectives. The position would be active in improving the quality of care outcomes related to value-based contracts and similar efforts. The Clinical Resource Specialist will assist in providing clinical analysis in order to achieve Population Health goal and objectives.

Your responsibilities will include:
  • Provide clinical support and direction on the clinical programs associated with Population Health contracts.
  • Actively participate with community based initiatives related to care management and transitions of care.
  • Conduct clinical audits to improve quality outcomes related to Population Health measures.
  • Identify and implement performance improvement processes in collaboration with the Executive Medical Director of Population Health.
  • Evaluate results of performance improvement processes and recommend changes as required in collaboration with the Executive Medical Director.
  • Develop annual goals collaboratively with other members of the Population Health team to improve patient outcomes.
  • Participate in Population Health program development.
  • Develop a comprehensive understanding of Catholic Medical Center’s Population Health Strategic Plan for innovative payment programs/concepts. Provide the clinical input for quality and utilization improvement in these initiatives.
  • Maintain knowledge of external regulations involving nursing practice and actively assists with quality measure compliance. Works with Care Coordination lead to facilitate changes to ensure compliance with commercial and government contract requirements as it relates to quality measures and outcomes.
  • Work closely with Case Management and the Care Coordination team to carry out the transitional care management program.
  • Work with the care coordination team to identify and assess individual patient needs to address issues related to conditions and/or overall health that have an impact on the quality of care.
  • Assist in the design for patient education on chronic disease management and works collaboratively with care coordination to ensure the implementation of these education resources.
  • Using the nursing process, conduct prospective, concurrent, and retrospective review for uncomplicated and complex cases in order to provide recommendations to improve quality outcomes.
  • Provide recommendations to carry out transitional care management programs to avoid unnecessary hospitalization and/or facilitate timely discharges. Demonstrate expertise in clinical nursing practices. Act as a positive, professional role model and demonstrate adaptability to changing nursing trends.
  • Represent Population Health on select practice and hospital committees and task forces.
  • Maintain current knowledge of nursing practice. Provide direct nursing care, and supports staff when appropriate.
  • Utilize evidenced based practice and research to support and advance nursing practice. Use research to provide recommendations for clinical care in order to work collaboratively with Catholic Medical Center’s PPA.
  • Collect all data for performance improvement measures and coordinate reporting to the Quality Improvement Department.
  • Implement continuous performance improvement in patient care and provide leadership in practice and clinical decision-making through use of the EMR.
  • Provide recommendations for implementation and ongoing refinement of the communication process of patient information.
  • Perform similar or related duties as assigned or requested.
