Job summary

Saint Alphonsus Health System in Idaho is seeking a Clinical Resource Manager to oversee hospital case management, utilization review, and discharge planning for a defined patient group. You will collaboratively determine level of care, coordinate social services, support physicians and care teams, ensure regulatory compliance, and promote efficient resource use from admission to discharge.

Qualified candidates hold an RN license in Idaho or Oregon, a BSN, and at least 2 years hospital clinical

Qualifications

• RN license in Idaho or Oregon; licensure by endorsement if moving to Idaho from another compact state.
• BSN is required; a master's degree is preferred.
• A minimum of 2 years of varied hospital clinical experience is required.
• Experience in case management, home health, and/or the insurance industry is preferred.

Responsibilities

• Provides hospital case management, utilization review, and discharge planning for a defined patient group.
• Collaborates with physicians, nursing, and ancillary staff to determine level of care, treatment, and transition needs.
• Coordinates social services integration into patient care and discharge planning; maintains regulatory compliance.
• Supports the care team with education on requirements and promotes efficient utilization of resources from admission to discharge.
