• Associate or bachelor’s degree in nursing required.
• Active RN license in the applicable state.
• At least 1 year RN home health or hospice experience.
• At least 1 year RN management experience.
• Current CPR certification.

Responsibilities

• Oversee clinical operations and quality of patient care.
• Manage staff scheduling and payroll verification.
• Review service requests and determine patient eligibility for home care.
• Coordinate referrals with intake and admissions.
• Lead on-call processes and staff training.
• Investigate and resolve patient/family/physician concerns.
• Assist with plan of care development and revisions.
