  • The right person for this job will be very proficient in Microsoft Office products. You will need to be personable, friendly and well-versed with effective communication, attention to detail and customer service.


Essential Functions:

  • Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care and planning for discharge.
  • Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
  • Supervises paraprofessional and licensed professional team members.
  • Assures ongoing interdisciplinary assessment of the patient.
  • Assures the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Assures services that are ordered by the physician are provided as indicated in the plan of care.
  • Counsels and educates the patient and family in meeting healthcare related needs.
  • Assures timely and accurate clinical notes including addressing patient's progress towards goals.
  • Coordinates services including patient care and referrals (alongside Client Service Supervisor).
  • Responsible for the Quality Assessment and Performance Improvement (QAPI) program.
  • Responsible for the Emergency Preparedness training/exercises.
  • Follows accepted standards of nursing practice.
  • Uses clinical decision making to efficiently and effectively manage an assigned patient population.
  • Accesses personal health information ("PHI") necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
  • Other duties and tasks as assigned by management.


Minimum Education & Experience Requirements:

  • Licensed Registered Nurse in the State of South Dakota
  • Graduate of an accredited school for applicable discipline.
  • License is active and in good standing.
  • Three (3) years home care experience preferred.
  • Two (2) years management experience preferred.


Schedule:

  • Monday-Friday
  • 8:00am-4:30pm
  • On Call
  • Office setting
