Job summary

Avow Hospice in Fort Myers, FL is seeking a Registered Nurse to support referrals into the agency, gathering medical and financial data, meeting patients and families, and working with case managers and discharge planners to ensure smooth transitions from hospital or facilities to home. This role focuses on referral growth within the assigned territory.

The RN will educate admissions staff, coordinate data for admissions, and facilitate access to Avow’s services, while maintaining HIPAA

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Two years of acute care experience; hospice/home care preferred.
• Current Registered Nurse license; Florida driver's license.
• CPR certification required; CHPN preferred.

Responsibilities

• Identify referrals and assess patient eligibility for services.
• Coordinate data for admissions and maintain CRM documentation.
• Educate families about services and assist with financial applications.
• Liaise with physicians, facilities, and case managers to expand referrals.
• Ensure HIPAA compliance and confidentiality in all processes.

Job Summary

Working in an assigned territory, facilitates referrals to agency by gathering and interpreting medical and financial data, meeting with patients and families, and working with healthcare professionals (case managers, discharge planners) to ensure optimal transition from hospital or other facilities to home. Responsible for overall referral growth in assigned territory and facilities. Supports admissions staff by educating and facilitating patient’s access to organization’s services within assigned territory and outside assigned territory as requested.

Job Duties

1. Makes initial contact and physical, social and psychological and financial assessment of patients to determine appropriateness of organization’s services.
2. Self identifies options for payment, which are not necessarily identified by patient, providing resources as needed including assistance with financial application.
3. Determines needs and coping skills of the caregiver as it relates to caring for patient.
4. Interprets policies and acts as resource for case managers, physicians, hospital and community for effective public relations and marketing of services.
5. Demonstrates the ability to identify the best course of services based on disease process and its progression. Coordinates organization’s services and suggests the appropriate services to family and staff.
6. Initiates referral-to-admission process for each patient referred by facilitating required data, leveraging available resources to ensure data is complete and accurate.
7. As needed, provides accurate and thorough documentation either via phone, fax, or computer to office for additional data entry.
8. Accurately, timely and appropriately documents in organization’s CRM.
9. Ensures documentation is legible and comprehensive.
10. Maintains confidentiality and follows HIPAA regulations.
11. Handles patient problems as they relate to unmet expectations of organization’s services.
12. Effectively communicates with physicians who are non-supporters of organization’s services to bridge this gap and develop a more positive relationship.
13. Communicates with families regarding changes in care plan activities.
14. Conducts individual and group presentations on products and services in an informative, accurate, and professional manner. Responds to questions about products and services.
15. Successfully builds relationships with case managers, social workers, and other care providers.
16. Acts as liaison between physicians, skilled nursing facilities, private duty companies, home health companies, hospitals and rehabilitation providers communicating service enhancements, new products, new programs and service delivery.
17. Facilitates communication between Business Development and Hospital, Skilled Nursing Facility representatives, and other referral sources. Relates opportunities for recovery activities.
18. Keeps agency informed of facility changes regarding bed capacity, staff changes, new facility programs, and/or procedures.
19. Understands that teamwork is a crucial part of our business, and is able to work well with colleagues, no matter the circumstances. Using individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.
20. Utilizes current agency and/or department specific software to complete assignments.
21. All other duties as assigned.

Qualifications: Education/Experience

Graduate from an accredited school of nursing. BSN preferred and two years of experience working in an acute care setting; one year of home care and/or hospice experience preferred. Working knowledge of community resources. Ability to meet, collaborate and work efficiently with others in the community. Knowledge of entitlement programs.

Certificates, Licenses, Registrations

• Current Registered Nurse license.
• Current driver’s license state of Florida.
• Possession of a registered and insured motor vehicle.
• CPR required upon hire and actively maintained through employment.
• CHPN certification preferred.

Language Skills

Ability to read, analyze, and interpret common scientific and technical journals, legal documents and other reports. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to prepare educational presentations that conform to prescribed style and format. Ability to effectively present information to public groups, facilities, hospitals, and community.

Mathematical Skills

Ability to work with mathematical concepts such as add, subtract, fractions, percentages, and ratios.

Reasoning Ability

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret a variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
