Job summary

Pacific Health Group is seeking a Registered Nurse - Clinical Case Consultant to lead clinical guidance for the CalAIM Enhanced Care Management program. The role includes supervisory responsibilities for LVNs, field visits, and ensuring high-quality, member-centered care across interdisciplinary teams.

The position emphasizes coordination with health plans, hospitals, and community partners to improve outcomes, reduce avoidable utilization, and ensure ECM compliance.

Qualifications

• Active RN license in California.
• 2+ years of direct clinical experience in community health or related settings.
• Experience with Medi-Cal populations and complex medical, behavioral health, and social needs.

Responsibilities

• Provide clinical guidance to ECM care teams.
• Provide supervision and mentorship to LVNs and staff.
• Conduct field visits and shadow care team members.
• Ensure documentation meets Medi-Cal ECM standards.
• Collaborate with health plans, hospitals, behavioral health agencies, and community-based organizations to coordinate care.
