Job summary

Qualifications

4+ years in health care with 2+ years in managed care areas.
Active clinical licensure only if required by state contract or regulation.
Demonstrated knowledge of community resources.
Proactive and detail-oriented with strong communication.
Ability to work independently with minimal supervision and self-motivation.
Excellent time-management and prioritization skills.

Responsibilities

Provide lead level support to healthcare services team in care management and related functions.
Coordinate processes, delegation strategies and communications across teams.
Resolve day-to-day issues and escalate to leadership as needed.
Assist in training staff and maintaining adherence to policies.
Monitor workload and ensure compliance with guidelines and programs.
Share performance feedback with leadership and support corrective actions.
Collaborate to run daily reconciliation and maintain care monitoring tools.
Act as liaison to internal and external customers while maintaining confidentiality.
