Job summary

Helping Hand Nurse, LLC is seeking a dependable RN to join our clinical team as a Clinical Field Supervisor. This role focuses on in-home patient care, conducting assessments, ensuring accurate and timely clinical documentation, and supporting quality assurance initiatives.

The role requires traveling to multiple patient homes, coordinating with families, and adhering to HHN’s procedures. Training will be provided to ensure regulatory compliance and high-quality care.

Qualifications

• Must hold an active RN license.
• Proficient with computer systems and electronic health records.
• Home health nursing experience preferred.

Responsibilities

• Conduct in-home Start of Care (SOC) and recertification assessments within required timeframes.
• Perform QA visits to evaluate patient care and ensure compliance with clinical standards.
• Complete all assessments, visit notes, and required clinical documentation in HHN’s EHR.
• Review patient records for completeness and follow up on missing documentation or outstanding clinical requirements.
• Communicate and address changes in patient condition, safety concerns, and clinical issues.
• Travel to multiple patient homes throughout the week and manage an organized visit schedule.
• Participate in training and follow HHN’s clinical procedures and regulatory requirements.
