Job summary

Professional Healthcare Resources is seeking a dynamic Clinical Nurse RN to join our Clinical Team of Professionals in Maryland.

This role involves performing comprehensive assessments for home health care or hospice patients, coordinating care as a case manager, and ensuring evidence-based patient outcomes. Requires an active Maryland RN license and at least one year of nursing experience; home health experience preferred.

Qualifications

• Active RN license in the relevant state(s) required.
• Minimum of 1 year of licensed nursing experience preferred.
• Experience in community or home health preferred.
• Knowledge of nursing principles and standards as per regulations.

Responsibilities

• Perform initial and ongoing assessments for home health or hospice patients.
• Act as case manager, coordinating services with the interdisciplinary team.
• Determine levels of care and eligibility for admission/recertification.
• Provide refrerrals for specialized nursing procedures and symptom management.
• Document all clinical activities per agency policy.
• Communicate changes in patient condition to physician and obtain orders.
• Coordinate care with patient, family and home health team for cost-effective services.
• Evaluate outcomes through performance improvement activities and chart reviews.
• Supervise LPNs and paraprofessionals in home health or hospice care.
• Participate in care conferences and quarterly trainings.
• Plan discharge and bereavement follow-up after hospice death.

Job description

We are looking for a dynamic Clinical Nurse RN to join our phenomenal Clinical Team of Professionals!

Responsibilities

• Performs initial and ongoing comprehensive assessments of the client’s needs at appropriate time points for home health care or hospice patients
• Acts as a case manager, coordinating all patient/family services and prioritizing the patient needs with the members of the interdisciplinary team
• Applies specific criteria for admission and re-certification to care, as applicable, to establish appropriate levels of care and patient’s eligibility
• Initiates appropriate preventive and rehabilitative nursing procedures, and provides referrals to other services and disciplines as needed, specialized nursing skill (such as IV), and effective symptom management
• Accurately and completely prepares all clinical documentation per agency policy
• Identifies and communicates changes in the patient’s condition; informs the physician and obtains new orders as needed
• Maintains communication with the patient, family and other members of the home care team to ensure coordination of services and episode management that meets patient goals in a cost-effective manner
• Evaluates outcomes of care through performance improvement activities, clinical record review, and other assigned duties
• Supervises LPN/s and paraprofessionals providing home health care or hospice services
• Participates in care conferences, staff meetings and at least quarterly, in-service programs
• Plans for the discharge from service or bereavement follow-up after hospice death

Qualifications

• Must have a Compact RN License and/or Registered Nurse licensed in the state(s) of operation, required
• Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations, required
• A minimum of one year of licensed professional nursing experience is preferred;
• Experience in community or home health preferred
