Beaumont Home Health & Hospice is seeking an RN Clinical Field Staff Liaison to support field clinicians, oversee compliance with clinical processes, and ensure safe delivery of services. The role includes on-call shifts and collaboration with managers to maintain high-quality home health care.

The candidate should hold an RN license in the state of employment, with compact licensure within 30 days for participating states, and have at least 3 years of acute care RN experience; one year of home

Responsibilities

• Serve as a resource for field staff and physicians.

• Review clinical documentation per Medicare guidelines.
• Participate in in-service program development.

The CFSL assists in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management. The CFSL is responsible for all practices and duties within the scope of practice as outlined by the state.KEY RESPONSIBILITIESServe as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concernsFacilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to managerWork in the field with staff and patients as neededMaintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and familiesAssist with monthly Case Manager, Nurse and Therapy meetingsEnsure Start of Care complianceEnsure quality and safe delivery of home health servicesMonitor Home Health Aide supervisory visitsFocus on Outcome Monitors as assignedImplement quality improvement programs in the fieldCompletes field visits as neededParticipates in managing cost of good working with monitoring supply costs and visits per episodeKnowledgeable of CFSS workflowReview relevant reportsReviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesMaintains active RN license in state of employmentParticipates in on-call, evening/weekend shifts and provides patient care, per agency needsParticipates in in-service program developmentMaintains a professional appearance as a representative of the companyUnderstands, implements and practices policies and procedures for agency operationOther duties as assignedQUALIFICATIONSRegistered Nurse with current license in the state of employment.
