Job summary

National Health Care Associates in Bethel, CT is seeking an RN Clinical Evaluator to conduct comprehensive assessments of residents’ health, functional abilities, and care needs. You will drive personalized care plans and collaborate with interdisciplinary teams to ensure the highest standard of care for our aging residents.

The ideal candidate holds a current RN license, with experience in long-term care, discharge planning, and effective communication across care settings.

Qualifications

• Valid state RN nursing license.
• 3 years of clinical or case management experience in long-term care.
• Experience in discharge planning and marketing activity.
• Compassionate and empathetic approach to patient care; interest in aged/chronically ill needs.
• Strong clinical skills and critical thinking abilities.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a multidisciplinary team.

Responsibilities

• Perform comprehensive assessments of physical, mental, and emotional health to determine appropriate care setting.
• Evaluate functional abilities and care needs through detailed observations and assessments.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Develop and maintain positive relationships with area hospitals and community agencies.
• Establish and maintain relationships with social workers, case managers and other contacts in healthcare/payor community.
• Educate residents and families about facility services, procedures, and care expectations.
• Monitor and reassess residents' progress and update care plans accordingly.
• Advocate for residents' well-being and coordinate with healthcare providers to address needs.
• Document assessments and care plans accurately and timely.
