Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

• Collaborate with the interdisciplinary team to assess patient status and implement and monitor the progression of care
• Perform care coordination and patient education
• Administer medications and treatments as directed by provider/physician staff
• Monitor patients throughout treatment and document responses to treatment
• Mobilize resources in complex cases to maximize patients' control and participation over their recovery
• Integrate assessment and diagnostic information with intuition to foresee potential healthcare needs and anticipate patients' needs
• Ensure effective handoff communication, including reporting on situations most likely to develop and problems awaiting the patient
• Display the ability to oversee the entire unit and self-motivate to adjust staffing patterns as needed
• Plan and provide individualized comfort measures utilizing evidence-based and innovative approaches
• Participate in performance improvement activities at the unit level
• Perform any other related duties as required or assigned

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification
• Clinical nursing experience in a dialysis setting is preferred

Rate: $45.00 - $55.00
