Job summary

Humboldt Park Health in Chicago, IL seeks an RN Clinical Coordinator to support behavioral health nursing leadership. This full-time, on-site role schedules 12:00pm-8:30pm or 1:00pm-9:30pm with every other weekend.

You will provide clinical guidance, conduct audits, mentor frontline staff, and help ensure patient safety and regulatory compliance across the unit.

Qualifications

  • Requires an Associate Degree in Nursing; BSN preferred.
  • Current IL RN license, BLS and ACLS certification.
  • CPI certification within 6 months of hire.

Responsibilities

Schedule: This is a full time position expected to work 12:00pm to 8:30pm or 1:00pm to 9:30pm Mon -Fri with every other weekend required.

Essential Duties and Responsibilities
  • Provide clinical and operational support to the Behavioral Health Nurse Manger and Director in maintain safe , effective , and regulatory -compliant patient care and unit practices.
  • Supports frontline nursing and mental health staff by providing clinical guidance, reinforcing established workflows, and serving as a resource for questions related to patient care, documentation, safety, and behavioral health practices.
  • Promotes a safe, therapeutic, recovery-focused environment consistent with Behavioral Health standards and trauma-informed care principles.
  • Conducts routine clinical and documentation audits, including patient records, nursing assessments, medication documentation, treatment plans, observation documentation, suicide precautions, and other high-risk patient records.
  • Monitors restraint and seclusion documentation and compliance, identifying gaps and escalating concerns to the Nurse Manager for follow-up.
  • Audits compliance with patient safety practices, including observation requirements, suicide precautions, rounding, falls prevention, patient identification, medication safety, and other Behavioral Health safety measures.
  • Supports regulatory and survey readiness by routinely reviewing unit practices and documentation for compliance with applicable hospital standards and regulatory requirements, including Joint Commission, CMS, and IDPH expectations.
  • Assists with quality improvement initiatives through data collection, validation, trend identification, and follow-up related to Behavioral Health quality metrics.
  • Supports the Behavioral Health quality dashboard by assisting with data collection, monitoring trends, identifying areas of opportunity, and communicating findings to the Nurse Manager.
  • Provides education to frontline staff related to identified clinical, documentation, safety, and regulatory gaps, in collaboration with the Nurse Manager and Educator.
  • Assists with the development and implementation of unit-based education, competency activities, case studies, and quality improvement initiatives.
  • Participates in interdisciplinary collaboration with providers, social work, therapy, case management, quality, and other departments to support safe and coordinated patient care.
  • Assists with patient and family concerns by addressing clinical concerns within the scope of the role and escalating unresolved concerns to the Nurse Manager or appropriate leader.
  • Rounds on the Behavioral Health units to identify environmental, clinical, safety documentation, and workflow concerns and communicates identified opportunities to the Nurse Manager.
  • Supports implementation of policies, procedures, and evidence-based practices and assists with monitoring staff adherence to established standards.
  • Assists with onboarding and orientation of newly hired Behavioral Health nurses and support staff by serving as a clinical resource and reinforcing unit expectations.
  • Identifies trends in clinical practice, documentation, safety events, and staff education needs and collaborates with the Nurse Manager to develop appropriate interventions.
  • Participates in safety event reviews, root cause analyses, and performance improvement activities as assigned.
  • Maintains appropriate professional boundaries and escalates employee performance, conduct, attendance, staffing, and disciplinary concerns to the Nurse Manager rather than independently managing corrective action.
  • Performs other related duties as assigned by the Behavioral Health Nurse Manager or Director of Nursing.

Role Boundaries: The Clinical Coordinator functions in a supportive and coordinating capacity. Final accountability for departmental operations, staffing, scheduling, employee performance management, disciplinary action, and overall nursing leadership remains with the Nurse Manager/Director of Nursing.

The Clinical Coordinator is expected to identify, communicate, coordinate, and follow up on opportunities for improvement, while escalating matters requiring managerial authority to the appropriate leader.

Position Requirements
  • Previous behavioral health experience
  • Associates Degree in Nursing. BSN preferred
  • Current IL RN licensure required.
  • BLS required.
  • ACLS required.
  • CPI required within 6 months of hire date.
