Job summary

Haven Hospice in Gainesville, FL is seeking a compassionate Clinical Community Liaison (RN) to build relationships with hospitals, nursing homes, and community providers, promoting hospice services and identifying patients who may benefit from care.

You will coordinate referrals with the admission team, support patients and families through transitions from hospital or home to hospice, and participate in outreach events to support market awareness.

Qualifications

• Registered Nurse with current licensure to practice in assigned states.
• Bachelor’s degree in Nursing preferred.
• Three years’ experience in clinical nursing; hospice experience preferred.
• Two years in compliance, internal audit, or paralegal preferred.
• Strong understanding of Medicare Conditions of Participation and medical documentation standards.
• Ability to manage highly confidential information.
• Analytical thinking with problem solving and data collection.
• Meticulous attention to detail and self-direction.

Responsibilities

• Meet with hospitals, nursing homes, and community providers to promote hospice care.
• Identify patients who may benefit from hospice and assess eligibility.
• Coordinate care between referral sources and the hospice admission team.

Qualifications

• Registered Nurse with current licensure to practice in assigned state(s)
• Bachelor’s degree in Nursing from an accredited program by the National League for Nursing preferred
• Three Years’ Experience In Clinical Nursing, Hospice Experience Preferred
• Two years of experience in compliance, internal audit, basic law or paralegal strongly preferred
• Excellent understanding of compliance, Medicare Conditions of Participation, and understanding of medical documentation standards
• Ability to manage highly confidential information
• Analytical skills with ability to define problems, collect data, establish facts, and draw valid conclusions
• Meticulous attention to detail
• Self-directed and ability to work with minimal supervision
