Job summary

Haven Hospice in Orange Park, FL is seeking an experienced RN Clinical Supervisor to oversee hospice staff, coordinate services, and guide clinical decisions in a patient-centered home-care setting.

The role emphasizes strong communication, leadership, and collaborative assessment to ensure high-quality end-of-life care. Candidates should have hospice or home care experience and a valid Florida RN license.

Qualifications

• Registered Nurse with current, unencumbered licensure to practice in the operating state.
• Graduate of an accredited school of nursing; BSN preferred.
• Hospice or home care experience required.
• Minimum of two years of management or supervisory experience preferred.
• CPR certification maintained per policy.
• Valid driver's license with insured vehicle in good condition.
• Demonstrates nursing judgment and excellent communication skills.

Responsibilities

• Review new patient referrals and determine care needs; assign appropriate hospice staff.
• Support and guide clinical staff through home visits, case reviews, and real-time consultations.
• Help develop patient care plans, set treatment goals, and prioritize care.
• Lead and participate in team case conferences to ensure coordinated, holistic care.
• Conduct regular chart audits and provide feedback for quality improvement.
• Participate in hiring, onboarding, and training of clinical team members.
• Support staff development and continuing education programs.
• Assist in policy development and uphold professional care standards.
• Engage in community outreach and promote the hospice’s mission and services.
• Contribute to quality improvement initiatives and organizational goals.

Qualifications

• Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation.

• Graduate of an accredited school of nursing; BSN preferred.

• Previous experience in hospice or home care setting required.

• Minimum of two (2) years of management or supervisory experience preferred.

• Current CPR certification, maintained in accordance with organizational policy.

• Must be a licensed driver with an automobile that is insured per state and/or organizational requirements and is in good working order.

• Proven ability to work effectively within an interdisciplinary team setting.

• Demonstrates excellent observation, nursing judgment, and communication skills—both verbal and written.

• Understands the hospice philosophy and the emotional, clinical, and ethical considerations surrounding end-of-life care.

• Knowledge of pain and symptom management principles.

• Adheres to accepted professional standards and practices.

• Commitment to clinical excellence and thorough documentation.
