Job summary

The Christ Hospital Home Health Care is seeking a diligent RN Case Manager to lead patient assessments and care planning in home health settings. You will coordinate multidisciplinary teams, educate patients and families, and ensure compliant documentation and regulatory adherence.

Preferred candidates have 1+ year of RN experience, including acute care, with strong communication and compassionate patient care. This role includes on-call rotation and requires an active RN license in the state.

Qualifications

• One year of RN experience in acute care; home health experience desired.
• Ability to communicate effectively with patients, families, physicians and care team.
• Compassionate, patient-focused and able to prioritize in a fast-paced home health setting.
• Knowledgeable about nursing best practices and clinical decision-making.
• Ability to document accurately and comply with regulations.

Responsibilities

• Oversees care from start to end of episode.
• Update orders in patient chart as needed.
• Notify physician of changes in patient condition.
• Coordinate multidisciplinary care and supervise aides and LPNs.
• Provide skilled nursing care as ordered by physician.
• Perform assessments and develop plan of care.
• Educate patients and families on care plans.
• Document all treatments and assessments per guidelines.
• Maintain active RN license and participate in on-call rotations.
