Qualifications

• Active, unrestricted RN OR SW License in MN is required.
• 5+ years of clinical experience.
• 1–2+ years supervisory experience of professional staff; remote/home office experience considered.

Responsibilities

• Provide direct supervision to the care coordinators and mentor team members.
• Educate the team on using membership information and tools to coordinate care.
• Manage care coordination for assigned member panel and ensure regulatory compliance.
• Monitor team performance, audits, and workloads; adjust caseloads as needed.
• Participate in on-call program and support training for new coordinators.
