Job summary

Prominence Health is seeking a Clinical Manager to oversee the Care Management program, D-SNP CM, and Transitions of Care. The role includes staff supervision, policy development, and ensuring CMSA standards compliance.

You will lead a team of RNs, social workers, and support staff to assess health needs, create and monitor care plans, and coordinate services with providers. The ideal candidate has RN licensure in applicable states, CCM/CPHQ or credentialing plan, and at least five years of

Qualifications

  • RN with a BSN or MSN required.
  • CCM or CPHQ certification (or ability to obtain) within required time.
  • Minimum five years of management experience in a managed care environment.
  • Knowledge of Milliman Care Guidelines preferred (recent within 3 years).
  • Minimum five years clinical nursing in hospital settings; travel up to 25–50%.

Responsibilities

  • Oversee day-to-day management of the Care Management program and Transitions of Care.
  • Supervise staff, ensure training and deployment to support Prominence Health Plan members.
  • Develop policies, workflows, and processes for comprehensive Care Management services.
  • Collaborate with PCPs and other providers to implement and adjust treatment plans.
