Pay Range: $70,000 - $75,000 + $2,000 Sign-on Bonus

  • Current Registered Nurse (RN) license in good standing
  • Graduate of an accredited nursing program
  • Home health, pediatric, or office experience is a plus

The Position:

We are seeking a full-time Clinical Care Manager with experience in an office setting, healthcare profession, or related field. Responsibilities include but are not limited to:

  • Ensuring direct care staff provide compliant care
  • Training and guiding staff on orders for disciplines, treatments, and services per agency policy
  • Ensuring referrals lead to timely start of care or are properly documented for delays or non-admissions
  • Managing a caseload of clients effectively
  • Developing and implementing treatment plans, setting realistic goals with patients, re-evaluating, adjusting care, and assessing discharge potential in consultation with physicians or care providers
  • Completing supervisory and assessment visits as per agency policy and regulations, with proper documentation
  • Coordinating care between staff and external providers
  • Acting as liaison with physicians, hospitals, and other agencies regarding patient care
  • Modeling professional behavior and maintaining a positive attitude
  • Orienting and verifying competencies of direct care staff
  • Supervising newly hired clinicians and ensuring all staff supervision is documented
  • Providing guidance for problem resolution in patient care
  • Reporting as required and participating in staff orientation, training, and quality programs
  • Providing training on topics mandated by state licensure, regulations, and policies
  • Participating in on-call duties, completing logs, and ensuring follow-up
  • Contributing to agency evaluations and reports as assigned
